Our model of care is designed to provide flexible, personalised mental health support across hospital and community settings.
Treatment recommendations are made in consultation with the treating psychiatrist, with care pathways that can be accessed directly - without requiring inpatient admission.
We support clients at every stage of their journey, ensuring the right level of care at the right time. This includes overnight hospital stays, structured day programs, and community-based care such as telehealth and in-home support.
Our Admissions and Assessment Coordinators provide pre-admission screening and risk assessment, working closely with referring clinicians and admitting specialists to determine suitability and facilitate timely access to care.
Dedicated care coordinators support clients throughout their journey, ensuring smooth transitions between inpatient, outpatient, and community settings, as well as ongoing support following discharge.
Overnight stays are tailored to individual needs, with length of admission determined collaboratively between the client and their treating psychiatrist.
