About Community Care

What is Community Care?

Community Care is a flexible treatment option where we bring hospital-level assessment, monitoring and therapeutic support to the client.

Care is delivered in the client's home or another agreed safe location, allowing them to stay connected to their daily life while still receiving close clinical support from our team.

Who is it for?

Community Care may be suitable if:

  • You would benefit from intensive support but prefer to remain at home
  • You have stable accommodation and a supportive environment
  • You live within a suitable distance of the hospital for home-based care
  • You are able to engage safely with treatment in a community setting
  • You are stepping down from an overnight admission or day program
  • You require structured support as an alternative to inpatient admission

What to Expect

With Community Care, clients can expect:

  • Regular visits from our clinical team in their home or agreed location
  • A combination of face-to-face and telehealth support
  • Ongoing assessment, monitoring and therapeutic interventions
  • A structured treatment plan tailored to individual needs
  • Available across all seven days of the week, as arranged with the treating psychiatrist.

Before any home visits begin, we may complete a telephone check-in to ensure the home environment is safe for clinical staff and suitable for treatment delivery.

If needed, care can be delivered in an alternative agreed location, or a treatment plan may be adjusted in consultation with the treating psychiatrist.

How Community Care is Delivered

Care is guided by the treating psychiatrist and coordinated by the nursing team.

Depending on individual needs, care may include:

  • Individual therapy and clinical support
  • Medication management and monitoring
  • Psychiatric review via home visits or telehealth
  • Support from allied health professionals as required

This ensures treatment remains structured, safe and responsive to individual needs.

When Community Care is Used

Community Care can be used:

  • As an alternative to an overnight admission
  • As part of discharge and recovery planning following admission
  • Alongside day programs for additional support

This flexibility allows care to be tailored to what best supports each stage of recovery.
